- Register Online. Have your Visa or MasterCard ready and click here to register on-line.
Please be sure to click the "Submit" button only once when using your credit card to pay for your registration/membership. Clicking the "Submit" button more than once will cause duplicate charges to your credit card.
- Register by Mail. Complete the printable form and mail it to 6000 E EVANS AVE #3-205, DENVER CO 80222. You may pay by check, money order or with a Visa or MasterCard.
- Register by Fax. Complete the printable form. Please be sure to indicate a form of payment. If paying by credit card, be sure to include your credit card information. Fax it to 303-758-9616.
Conference Registration Confirmation
A confirmation will be emailed to you at the email address provided on your registration form.
Cancellations
Notify the Colorado Summer Orthodontic Meeting at (303) 758-9611 immediately if you must cancel your conference registration. Cancellations called, faxed, or e-mailed in and/or postmarked by May 15, 2009, will be eligible for a registration refund.
